What Is GeneSight Testing? What It Can (and Can't) Tell You About Your Medication

Have you ever been told you’re sensitive to medications or supplements or noticed that you seem to react differently than other people? Maybe a medication caused unexpected side effects, seemed to have the opposite effect, or simply didn’t work the way you hoped. If you have tried a medication for depression, anxiety, or ADHD that didn't help, or the side effects made you want to stop, you already know the hardest part of psychiatric care: finding the right medication can take time, and the waiting is discouraging. Somewhere along the way, you may have heard that a genetic test can help. I'm Charles Scribner, a psychiatric nurse practitioner in Brea, and provide GeneSight testing and use it in my practice. Here is what the test actually is, what it costs, and the honest version of what it can and cannot tell you.
What is GeneSight testing?
GeneSight is a pharmacogenomic test, which means it looks at how your genes may affect the way your body handles medication. It analyzes your DNA for how you may process or respond to more than 60 medications commonly prescribed for depression, anxiety, ADHD, and other mental health conditions [1].
The idea behind it is simple. Two people can take the same dose of the same medication and have very different experiences, and part of that difference is genetic. Some people break a medication down so quickly that a standard dose may not build up enough to help. Others break it down so slowly that a standard dose acts more like a high one, which is where side effects often come from.
How does the test work?
The whole thing runs on a cheek swab. You can do it in the office in under a minute, or I can have a kit sent to your home if we're working together by telehealth. The sample goes to the lab, and results typically come back to me in about three days [1].
The report sorts each medication into one of three categories: use as directed, moderate gene-drug interaction, or significant gene-drug interaction [1]. In plain terms, it flags which medications your body is likely to handle normally and which ones may need a different dose or a closer conversation before we consider them.
What can the results tell you?
The report gives us a head start on the metabolism question. When I review your results with you, I'm looking at things like whether your body processes a certain class of antidepressants quickly or slowly, and whether a medication you already tried sits in one of the flagged categories. When someone tells me a medication "did nothing" at a normal dose, and their report shows they metabolize it rapidly, that history suddenly makes sense.
That context is genuinely useful. It can help explain past experiences, and it gives us more information when we choose what to try next.
What can't it tell you?
I want to be direct about this, because some of the marketing around genetic testing goes further than the science does. GeneSight does not tell us which medication will work for you. No test can do that today, and the FDA has specifically cautioned against tests that claim to predict how well you will respond to a specific medication [2].
The best study we have is a large randomized trial called GUIDED, which tested this approach in over 1,100 people with depression who had already failed at least one medication [3]. The people whose care was guided by testing had higher rates of response and remission, and the benefit was largest for people who had been taking a medication their genes flagged. So the evidence is real, and it is strongest for people who have already had a rough experience with medication, rather than as a crystal ball for someone starting fresh.
In my practice, the report is one input. Your history, your symptoms, your goals, and what has and hasn't helped before all carry at least as much weight. The test informs the conversation, and the decision is still ours to make together.
How much does the GeneSight test cost?
This is usually the second question, and the answer is more reasonable than most people expect. According to GeneSight, 98% of patients pay $330 or less out of pocket, and Medicare and Medicaid patients typically pay nothing for covered tests [4]. If your cost could ever be more than $330, GeneSight contacts you before processing the test, and interest-free payment plans are available [4]. You will not get surprised by a mystery lab bill months later.
Who should consider testing?
Testing tends to earn its keep in a few situations:
You have tried one or more medications for depression, anxiety, or ADHD without much relief
Side effects have made you stop a medication, or made you afraid to start one
You're beginning medication for the first time and want more information going in, with the understanding that the strongest evidence is for people who have already had a failed trial

Questions I hear a lot
Does insurance cover it? Often, yes. The provider is able to submit documentation that can help with insurance approval to cover the cost most times. Medicare and Medicaid typically cover it fully, and with commercial insurance the vast majority of patients land at $330 or less [4].
How long does it take? The swab takes under a minute, and results usually reach me in about three days [1].
Is this a diagnosis? No. The test says nothing about whether you have depression, anxiety, or ADHD. It only speaks to how your body may handle certain medications. Diagnosis comes from a real evaluation with a clinician.
