Spravato (esketamine) is an FDA-approved nasal spray for adults with treatment-resistant depression, and for depressive symptoms in adults with major depressive disorder with acute suicidal thoughts or actions. You take each dose yourself under supervision in a certified healthcare setting, then stay for monitoring for at least two hours. At Peaceful Presence, Spravato is offered in person at the Brea office.

What Spravato is

Spravato is a nasal spray form of esketamine. It works differently from most antidepressants. For treatment-resistant depression, it is used together with an oral antidepressant.

Because Spravato can cause sedation, dissociation (feeling disconnected or unusual), and higher blood pressure for a short time after a dose, the FDA allows it only in certified healthcare settings under a safety program called REMS. There are no take-home doses.

Who Spravato may be right for

Spravato is not a first treatment. It is one option for adults when other treatment hasn't helped enough.

Adults with treatment-resistant depression

If you've tried antidepressants and your depression hasn't improved enough, Spravato may be an option to discuss.

Adults who can plan around the visits

Each session includes at least two hours of monitoring, and you cannot drive until the next day after a restful sleep, so you need a ride home.

How I approach Spravato

Before Spravato, I complete a full psychiatric evaluation and review your past treatments, blood pressure, health history, and any reasons it may not be safe for you.

On treatment days, you take the spray yourself under supervision. We check your blood pressure and how you feel, and you rest in the office until it is safe for you to leave. We track your progress and adjust your overall plan together.
